Surgery remains the standard approach to glioma. Brain surgery (a.k.a. craniotomy) is typically performed under general anaesthesia with the balance of maximal resection of gliomas and maximal preservation of critical brain functions. At NUH, we specialise in the resection of gliomas via the Awake Brain Surgery (or awake craniotomy), where a suitable patient is deliberately awake for a portion of the procedure. Advanced asleep brain mapping techniques are also used to optimize patients' outcomes.
The endoscopic endonasal (a.k.a. transsphenoidal) surgical approach uses the nostrils as a natural surgical corridor to remove pituitary adenomas, and some midline skull base and brain tumours. This endonasal route is the preferred surgical approach for a large majority of pituitary adenomas and Rathke's cleft cysts, as well as some of the craniopharyngiomas, clival chordomas and midline meningiomas.
Our Paediatric Neurosurgical Service, as part of the Division of Neurosurgery, provides care for our paediatric neurosurgical patients. The conditions that are commonly managed include brain and spinal tumors, hydrocephalus, refractory epilepsy, spinal dysraphism, tethered cord and head injury. Our comprehensive clinical/surgical management and outcomes are benchmarked against top academic paediatric neurosurgical units around the world.
The Gamma Knife Radiosurgery is a form of stereotactic radiosurgery that uses computer-guided planning and radiation to treat primary brain tumours, brain metastasis, arteriovenous malformation, trigeminal vascular abnormalities, and other lesions in the brain. It may be used as the primary or secondary mode of treatment for certain brain lesions under the circumstances where surgical removal of the lesion(s) is impractical or undesirable.
