How to apply for a Medical Report?
Payment is required in advance for all medical report requests. Requests cannot be processed until payment has been received.
Before submitting your application, please review our cancellation policy.
1. For SingPass users: You can apply for a medical report via the NUHS App (under “Medical Report Request” module).

2. If you are applying on behalf of your family member (parent/ child), please add them as a dependent and apply under their profile.

3. Please submit an enquiry using this form if your request meets the following criteria:
- You are not a Singpass user
- You are applying on behalf of a patient who is unable to provide consent
- The medical report is not available on the NUHS app
- You have a payment enquiry regarding your request
- You are seeking a clarification on a previous medical report request

Consent Forms for Request of Medical Types not available in NUHS App
All requests for medical reports must be accompanied by an Application & Consent for Release of Medical Information (Form A). Please ensure that Form A is completed and attached to the enquiry form.
Additional consent and declarations may be required for verification purposes:

Cancellation Policy
There will be no refund provided upon cancellation of investigation results, medical report certification, duplication of medical certificate and discharge summary.
For cancellation of all other reports, an admin fee of one-third the report price will be levied. No cancellation will be allowed once the report has been written by the doctor.
