If you would like to request for a physical bill for claims purposes, please fill in this form. The bills you have requested for will be mailed to the patient's registered address via post within 7 working days. The form will be voided if it is incomplete or if it contains inaccurate information.
If you are unable to use the OneNUHS app, you may also pay your bills online at https://eservices.healthhub.sg/payments.
