Advance Care Planning (ACP) is the process in which you communicate and document your wishes regarding your future medical care. ACP serves as a guide for your healthcare team and loved ones in delivering treatment and care options that are consistent with your wishes if you are in a medical crisis and unable to communicate your thoughts.

ACP allows you to choose someone you trust to speak on your behalf when you are unable to do so yourself i.e. Nominated Healthcare Spokesperson (NHS).
