NUH Transitional Care Programme

Overview

Most patients get well and go home after being treated at the hospital. However, some patients will need further medical care after discharge.

The time when patients leave the hospital to recover and get better at home can be stressful for both patients and family members.

To help patients transit smoothly from the hospital to their home, the healthcare team at NUH has developed a programme called the NUH Transitional Care Programme. This team consists of doctors, nurses and allied health professionals who will provide continued care in the comfort and privacy of the patient's home.

Patients, family members and caregivers will be taught on how to care and manage the recovery and/or rehabilitation.

Services:

  • Consultation and assessment by a team of doctors, nurses and allied health professionals
  • Patient education and psychological support
  • Physiotherapy, occupational and speech therapy
  • Medication management
  • Referral to appropriate community resources
