Birth Registration

Birth registration service is now available to parents who wish to register their newborns at NUH. Registration should be done within 42 days of the child's birth but parents are encouraged to register within 14 days.

Registration of newborns can be facilitated at the following Patient Service Centre 3.

Required Documents

Parents /Informants are required to produce the following documents for birth registration:

  1. Notification of Live Birth form (BD 50) issued by a medical practitioner or midwife
  2. Original identity cards of both parents. Parents who are non-Singaporeans are required to produce their foreign identity cards/passport/entry permits
  3. The parents' original marriage certificate
  4. Letter of authorisation from the child's parents if registration is done by another person

Please be informed that only the informant is allowed to sign on the child's birth certificate.


Child's Name

Please choose your child's name carefully and have it ready at the time of birth registration. Ethnic characters of the child's name in Chinese, Jawi and Tamil can be included in the child's birth certificate. This is optional but parents / informants must ensure the accuracy of the ethnic characters furnished. Parents / informants of birth will have to write the ethnic characters in the birth certificates themselves. If you are not able to do so, please bring someone to write it on your behalf.

Cost

The fees payable for birth registration at NUH is S$30.00 (inclusive of GST). This amount includes S$18.00 birth registration and a S$12.00 administrative fee. Payment can be made either in cash, NETS or credit cards.
