Documents to Bring

Please bring along the original of the following documents for your admission registration:

1. Admission and Financial Counselling Form
2. Permanent Resident documents/NRIC/Birth Certificate (for patient under 15 years old)
3. Valid notification letter from work permit department (for temporary work permit holders)
4. Passport/Employment Pass/Work Permit/Department Pass (for non-Singapore citizens)
5. Insurance card/Employer's Guarantee Letter (if you are settling your bills through insurance or employer's coverage)
6. Medical Benefits documents, eg. Civil Service Card (CSC), Hospitalisation Identity Card (HIC) or Blood Donor Card.
