We are committed to a long term national effort to fight asthma by taking pole position in the Singapore National Asthma Program (SNAP). The SNAP adopts a collaborative multi-faceted strategy, with our partners in the community, to improving asthma control for our patients. This approach includes intensive education for our patients with assistance from our Asthma Care Nurses and modern medications. The key tools which enable us to improve asthma control effectively in SNAP are accurate objective self-assessment of disease control with the Asthma Control Test (ACT) and individualised written asthma action plans.
