
LIVE Programme
ControL of Coronary RIsk Factor InitiatiVE (LIVE) Programme
This is the first such strategic partnership between the National Healthcare Group, solo and group GPs for patients with chronic disease and will have potential as a template for the management of other chronic conditions.
Ischeamic heart disease is a major health concern in Singapore. It is the second leading cause of death in Singapore, accounting for 19% of all deaths (2003).
Objective of LIVE Programme
The objective of the LIVE programme is to ensure all patients admitted to The Heart Institute with coronary heart disease-related diagnoses achieve optimal control of hypertension and hypercholesterolaemia in the most cost-effective way. The cost-effectiveness of these new medications in achieving target levels will be tracked; and appropriately step-down care of stable patients after 6-12 months to GPs.
Programme Components
While patients are in the hospital, they would :
- have received education regarding the importance of risk factor control;
- have achieved LDL-C and blood pressure targets;
- be on a list of medications titrated appropriately.
Upon discharge to GPs, there would be :
- a standard of care;
- a means of communication with a consultant cardiologist for enquiries;
- a system of referring patients back to hospital at subsidised rates.
Access to Database / Information on Patients under LIVE Programme
GPs partners under the LIVE Programme will need to allow the LIVE Programme Team (Programme Director, Executive and Case Managers) to have access to the patients’ medical information regarding BP and cholesterol control and medication utilisation.
