Due to its high prevalence, heart failure has been identified as a priority area for disease management in Singapore. In USA, comprehensive heart failure disease management programmes have shown reduction in re-hospitalisation and an improved quality of life for patients. With the Heart Failure Disease Management programme in NUH, we have reduced the number of unplanned re-admissions of patients and improved the quality of care through our multidisciplinary case management approach.
